Columbia County Health Department offering walk-in clinic for back to school vaccinations

COLUMBIA COUNTY, Ga. (WJBF) – The Columbia County Health Department is offering a walk-in clinic to get children vaccinated for school.

The clinic runs through August 5 and again August 8-12 from 8 a.m. to 3:30 p.m. at the Health Department, located at 1930 William Few Parkway.

 

Requirements for School 4-6 Years

  • DTAP series
  • Hepatitis B series
  • Polio series
  • MMR (2 doses)
  • Varicella (2 doses)
  • Hepatitis A (If born on or after 01/01/06, 2 doses)

Requirements for School K4 & K5

  • Hearing, Vision, Dental Screening and Nutrition Screening (form 3300)

According to the health department, students transferring to Georgia from another state must meet state vaccine requirements to attend school. Parents must bring their child’s immunization record to the health department to be evaluated by the nurse for vaccine requirements to receive form 3121 for school entry.
